Appointment Scheduling--New Process

1. To schedule an appointment, complete the VFC Eligibility and Child Screening Form through our NEW portal.

2. Upload your child's Immunization Records--you may take a picture of your child's immunization records and text to 847-474-9396 or email to HHS@vohe.org 

3. Upload your child's Medicaid card or proof of underinsured status.

Once eligibility is confirmed, our nurses will call you to discuss vaccines and schedule an appointment.

If you cancel, change or miss your scheduled appointment with less than 24-hour notice, we reserve the right to charge a $25.00 out-of-pocket fee.

Health and Human Services offers low-cost immunizations through the Vaccines for Children (VFC) Program for children 2 months to 18 years at Village Hall. This program provides vaccines to those who are uninsured, underinsured, or qualify for Medicaid. As of October 1, 2023, the cost for each vaccine is $20 for all communities, or free with Medicaid.

Our practice believes that children should receive all recommended vaccines on time. Vaccines are a safe and effective way to protect children’s health and wellness in our community. We provide the latest comfort techniques for children receiving immunizations.

Vision and Hearing Screenings are required yearly for 3—5-year-old children who attend a preschool or daycare in Illinois. Screenings are conducted by Illinois Department of Public Health certified audiovisual technicians from the Village of Hoffman Estates.

• Cost is $5 per screening.

Please Note: Screenings are not a substitute for a complete eye or ear exam by a doctor. 

The Village of Hoffman Estates will host a Mobile Dental Clinic for all children from birth to 18 years old and college students during the month of July. Dental services provided are cleaning, exam, sealants and fluoride treatments. 

Dental exams are required for children entering kindergarten, grades 2, 6, and 9. These preventative services will satisfy the Proof of Dental Exam requirement for your child’s school. You will receive the certificate upon completion of the exam.

Medicaid, Medicaid Managed Care and private insurance are accepted. No dental insurance? Cleaning and exam are $20, Fluoride is $29, Sealants on premolars is $35 per tooth with cash, checks, debit/credit card and payment through Care Credit accepted.

The Mobile Dental Clinic is open to all communities. Appointments are required.

HHS offers the following vaccines Immunizations for adults 19 years and older:

• Hepatitis A
• Hepatitis B
• Twinrix (Hepatitis A and B combined)
• Tdap (tetanus, diphtheria, pertussis-whooping cough)
• Flu/High-Dose Flu

Health and Human Services now offers vaccines through the IDPH 317 Program for eligible adults who are 19 years and older.

Mobile Health Clinics are held at locations around the Village. HHS nurses provide blood pressure monitoring, blood sugar, iron, medication and nutritional consultation.

Tuberculosis skin testing is available at the Village Hall through the Department of Health and Human Services.

• The cost for this service is $20. All communities are welcome.
• Appointments are required.  Call 847-781-4850.

Please note: TB tests are administered on Mondays and results are read on Wednesday.

To schedule an appointment for a TB skin test, please complete the TB Skin Testing Form in our NEW portal.

Once eligibility is confirmed, our nurses will call you to discuss the TB skin test and schedule an appointment.

If you need a TB blood test, call Cook County Health at 708-687-7200 or call your local pharmacy.

Cholesterol Testing

The Hemoglobin A1C test is a simple finger stick to find out if you are at risk or have diabetes by measuring your average blood sugar levels over the past three months.  Fasting is not required. The test takes 6 minutes, and the cost is only $15.To schedule an appointment, call 847-781-4850.

• Wheelchairs--$20 rental fee
• Walkers (with or without wheels) --$10 rental fee        
• Crutches--$10 rental fee
• Canes--$10 rental fee
• Knee Scooter--$20 rental fee
• Reachers--$5 rental fee

Residents can have the equipment on loan for up to 8 weeks. The rental fee is non-refundable. Complete the Lending Closet Form and email with your driver's license to HHS@hoffmanestates.org. Please bring the completed Payment Authorization Form with you when picking up equipment. For more information, call 847-781-4850.

HHS does not loan out equipment to residents of other communities.

Activities include a nutritious lunch program, bingo, sewing club, book club, health clinics, trips, and entertainment. The Barn is located at 231 South Civic Drive, Schaumburg.
