Qualifying Life Event
A qualifying life event gives benefit eligible employees a special enrollment period to make changes to their benefits, outside of open enrollment. These changes are effective immediately pending submission of all required documentation within 30 days of the event. Benefit changes must be consistent with your life event/status change.
Examples of qualifying life events include marriage, divorce, birth/adoption of a child, loss of other coverage, entitlement to Medicare, etc.

Empower Wellness Screening Incentive Program
Don’t miss out on a chance to win a $1,000 or $500 raffle prize!
We are excited to announce that the Village will once again provide wellness screenings for employees and spouses participating in Village health insurance plans, which will include a participation gift card (for employees only) as part of our wellness program. These screenings consist of a comprehensive wellness blood panel and an assessment tool that evaluates your current health status and lifestyle.
There will be three separate dates and locations for eligible employees and spouses to complete their blood draw.
- Thursday, October 3, 2024, 7:00am to 11:30am, Village Hall - Alexa Room – Benefits of Wellness Fair
- Friday, October 18, 2024, 6:30am to 10:30am, Police Department - Training Room
- Tuesday, October 29, 2024, 6:00am to 10:00am, Public Works – Lunch Room
To successfully participate in the Empower Wellness Screening Incentive Program, follow the steps below:
- Log into (or create) your Empower Health account.
- Returning participants: login and register at https://empower.health/HoffmanEstatesEHS
- New participants: create your account at https://empower.health/HoffmanEstatesEHS and enter client code: HoffmanEstatesEHS
- Complete the "Health Risk Assessment" in the portal.
- Register for a wellness screening to complete your blood draw.
- Employees unable to complete their blood draw at any of the scheduled events have the option of completing the wellness screening by submitting a completed Health Provider Form by March 31, 2025. Click here for instructions on how to download the Health Provider Form which is available in your Empower portal after completing the health risk assessment.
- Fire department employees who wish to use their next annual physical for their wellness screening, contact HRM for more information.
Incentive
Our goal is to have at least 75% of Village-insured employees participate in this year's program! Therefore, we need at least 230 insured employees to participate. Last year, the Village had a total of 203 employees participate in the program so this goal is very reachable!
The Village is very committed to this program and the more of you that participate, the higher the value of the gift card! Employees that successfully participate in the Empower wellness program will receive a gift card after the program conclude in Spring 2025.
- 125 employee participants - receive a $100 gift card
- 150 employee participants - receive a $150 gift card
- 230 employee participants - receive a $200 gift card
All active employees that successfully participate in the wellness screening program will be entered into a raffle for a chance to win a prize valued at $1,000 or $500!
Please note - the Empower wellness screening incentive program is available to employees, and their spouses, on one of the Village’s medical insurance plans. Gift cards are provided only to employees (not spouses) who (1) are active Village medical insurance participants at the time of distribution after the program concludes in Spring 2025, (2) complete the online Health Risk Assessment, and (3) participate in the blood draw (or submit a completed Health Provider Form).
