For Retirees actively enrolled in Medicare Part A & B

The Village of Hoffman Estates has chosen United American as your carrier for group retiree medical coverage because of their many years of experience in this market. The United American program works seamlessly with Medicare and covers most of your out-of-pocket expenses. Highlights of this plan include:

  • No paperwork or claim forms/electronic claims processing

The prescription drug plan being offered is a Medicare-approved plan insured Express Scripts Medicare®. Express Scripts contracts with the federal government to administer Medicare prescription drug coverage. You will use the Express Scripts network pharmacies to fill your prescriptions. Features of this plan include: 

  • A network of national pharmacy chains and independent pharmacies
  • Unlike other Medicare part D prescription drug plans, this plan will provide you with the generic and brand coverage through the “donut hole” coverage gap
  • Up to a 90-day supply through mail order service

As with any change in prescription drug carrier, you will need to obtain new mail order prescriptions from your physician to switch your medication to this plan 

BENISTAR Admin. Services, Inc. will be administering and servicing various aspects of your medical and prescription drug coverage. Your new ID card will have a toll-free Retiree Customer Service number to call with any questions you have about your prescription drug and medical coverage.

If you enroll in the prgram, you will receive a medical packet from United American/BENISTAR Admin. Services, Inc., as well as a prescription drug packet from Express Scripts Medicare (PDP), containing items pertinent to your new coverage. Included within this package, you will find your new medical and prescription drug ID card, policy certificate, evidence of coverage, formulary, and other plan information. Once you receive these items, please discard your current ID cards and use the new cards going forward.
