The Village of Hoffman Estates will once again provide wellness screenings for employees and spouses participating in Village health insurance plans. These screenings consist of a comprehensive wellness blood panel and an assessment tool that evaluates your current health status and lifestyle. A participation gift card will be given out in the Spring to employees only.

To successfully participate in the Empower Wellness Screening Program, follow the steps below:
| 1. Log into (or create) your Empower Health Account. Returning participants: sign in under "Welcome Back!" at https://empower.health/HoffmanEstatesEHS. New participants: create a new user account at https://empower.health/HoffmanEstatesEHS and enter client code: HoffmanEstatesEHS. Verify your email email address to activate your account. |
| 2. On the dashboard, click "Register" to schedule an appointment for an onsite event or to submit a Health Provider Form by 3/31/2026. Fire employees participating in a Village health insurance plan and would like to participate in the wellness screening program should simply provide verbal consent at their next annual physical and submit the gift card selection form at the time of consent. |
| 3. Complete the Empower Health Assessment on the dashboard. |
| 4. For onsite appointments, download and print one (1) copy of your requisition to bring to your screening (REQUIRED). |
Our goal is to have at least 75% of Village-insured employees participate in this year's program! Therefore, we need at least 230 insured employees to participate. Last year, the Village had a total of 210 employees participate in the program, so this goal is very reachable!
The Village is very committed to this program and the more of you that participate, the higher the value of the gift card! Employees that successfully participate in the Empower wellness program will receive a gift card after the program concludes in the Spring.
- 125 employee participants - receive a $100 gift card
- 150 employee participants - receive a $150 gift card
- 230 employee participants - receive a $200 gift card
Please note – the Empower wellness screening program is available to employees, and their spouses, on one of the Village’s health insurance plans, but the participation gift cards are provided only to employees (not spouses) who are active at the time of distribution after the program concludes in the Spring.

Employees and spouses participating in one of the Village's BCBSIL PPO plans are eligible to participate in the annual wellness screening program.

Yes! You have the option to complete your wellness screening by visiting your healthcare provider. Follow the steps below or within the Health Provider Form Information:

4. On the dashboard, click "Download Form" to print the Health Provider Form to bring to your healthcare provider.
